The 2006 fee is $50/player or $100/family.

Registration forms can be turned in at training.

Your child's age bracket is based upon what age they were as of January 1, 2006.
For example, if your child was ten on January 1, 2006, they are considered U12.

Please attach two current photographs (1"x1") per child,
a copy of their birth certificates,
as well as a copy of your medical insurance card.
Players are required to sign the registration
form in front of the one of the club's registrars.
